To assist EPB employees and retirees in completing the Direct Deposit form requested by EPB Payroll, your Credit Union is providing the below examples with forms. Please remember that you will NOT receive an actual check once you have elected to use Direct Deposit. Please check with the Credit Union if you have any transfers and splits to be sure these remain the same:
Please print the appropriate form, fill it out, sign it, and return it to the EPB Payroll Department.

If you want to deposit ALL of your paycheck into EPB Employees Credit Union, please use this form:

(Same as "net pay" folks)

If you want to deposit a fixed amount of money into another financial institution, AND you want to deposit the remainder of your paycheck into EPB Employees Credit Union, please use this form:
